• Address for home sample collection.

• Doctor’s prescription or referral, where applicable.

A booking request is not necessarily confirmed until Astha Pathology Lab or its authorized representative confirms the appointment.

We may contact you to verify booking details, confirm availability, request additional information, or reschedule an appointment.

4. Patient Responsibilities

Patients or their authorized representatives are responsible for:

• Providing accurate personal and health-related information.
• Following test preparation instructions, including fasting requirements where applicable.
• Informing the laboratory about relevant medical conditions, medications, allergies, or other circumstances that may affect sample collection or testing.
• Being available at the confirmed time for home sample collection.
• Providing access to the stated collection location.
• Following instructions given by authorized laboratory personnel.
• Reviewing the patient name and other details on the booking or report and promptly reporting any errors.

Failure to follow preparation instructions may affect the accuracy or interpretation of test results.

5. Sample Collection

Sample collection may be performed at our laboratory or at a location selected for home collection, subject to availability.

The actual collection time may be affected by traffic, weather, operational conditions, emergencies, incorrect address details, or other circumstances beyond our reasonable control.

Astha Pathology Lab may refuse, postpone, or repeat sample collection if:

• The patient is unavailable.
• The location is inaccessible or unsafe.
• Required preparation instructions have not been followed.
• The sample cannot be collected safely.
• The sample is insufficient, contaminated, damaged, improperly labelled, or otherwise unsuitable for testing.
• A prescription, identification document, or other required information has not been provided.

If a sample is rejected or cannot be processed, we may request a fresh sample. Additional charges may apply where permitted and communicated to the patient.

6. Test Reports

Laboratory reports are prepared based on the sample received and the testing procedures applicable to the selected test.

Reports may be delivered through:

• Email.
• WhatsApp or SMS notification.
• Online patient portal.
• Printed copy.
• Authorized doctor, clinic, or other approved channel.

You are responsible for providing accurate contact details and ensuring that you have access to the communication channel used for report delivery.

Reports should be reviewed by a qualified doctor or healthcare professional in the context of the patient’s symptoms, medical history, physical examination, and other relevant information.

7. Medical Disclaimer

Astha Pathology Lab provides diagnostic testing and laboratory reports. Our reports are not a substitute for medical consultation, diagnosis, treatment, or emergency medical care.

You should not:

• Self-diagnose based only on a laboratory report.
• Start, stop, or change medication without consulting a qualified healthcare professional.
• Delay urgent medical care because of a test booking or report.
• Treat a laboratory result as a complete assessment of your health.

If you experience a medical emergency, contact an appropriate emergency medical service or visit the nearest hospital.

Reference ranges and test interpretations may vary according to the test method, laboratory standards, patient characteristics, and clinical circumstances. A result outside or within a reference range does not by itself confirm or rule out a medical condition.

8. Test Availability and Changes

Astha Pathology Lab may change, suspend, or discontinue a test, package, service, price, collection location, or report-delivery method without prior notice where reasonably necessary.

We may also:

• Substitute a test with an equivalent or appropriate test where required.
• Request a repeat sample.
• Correct an administrative or reporting error.
• Reschedule a booking due to operational or technical reasons.
• Decline a booking if the requested service is unavailable or unsuitable.

Where a material change affects a confirmed booking, we will make reasonable efforts to inform the patient.

9. Pricing and Payment
